This article explains how duplicates are handled in Registrations to Review.
When an Event has more than 5,000 Registrations, no more Registrations will show under Registrations to Review - Potential Duplicates.
The only way to search for potential duplicates in this scenario is via an exported Excel file.

Searching for Duplicates via an Exported File
This section explains how to search for duplicates by using an exported Excel file.
Create a Registrations report with all desired fields, including Full Name.
Export the report to Excel.
Open the exported Excel file.
Select the Full Name column.
In Excel, choose Conditional Formatting > Highlight Cells Rules > Duplicate Values > Set a color.
This highlights all registrations with the same names.
Review the highlighted registrations to determine whether they are duplicates and take action accordingly.
