Submit a request
This page provides a form to submit a support request to the Certain Touchpoint Support Center.
Form fields
- Your email address: Text input field with a required indicator.
- Priority: Dropdown with options: "-", "Low", "Normal", "High", "Urgent". A link labeled "Request priority" appears near this field.
- Subject: Text input field with a required indicator.
- Event Name: Text input field. Please add your Event or Form Name. If your query is not related to a specific event, "N/A".
- Event Date: Text input field. Please add your Event date. If your query is not related to a specific event, please enter "N/A".
- Description: Text area. Please provide steps to reproduce, including browser used and attach screenshots if available.
- Attachments: File upload area with a placeholder "Add file or drop files here".
- Submit: Button labeled "Submit"
